What comes after the First Aid?

Once the casualty has been treated, what do you do while you wait for help?

When do you ‘stay and play’ or ‘swoop and scoop’?

The Prolonged Field Care course is designed to provide candidates with additional skills and knowledge to better prepare them for the unique challenges of providing long-term care to casualties in remote environments.

These challenges include:

1. A greater likelihood of injury or illness - because participants are likely engaging in more adventurous activities and / or in more challenging environments.

2. A greater severity of injury or illness - because of the nature of those activities and / or environments

3. The added complications of long-term care - because participants are likely to be further from definitive care in therms of time, distance and access.

We will also cover the medico-legal issues of medication, invasive techniques and the provision of medical care abroad. This course will NOT cover invasive surgical procedures but will increase the participants capabilities and competencies in a relatively short period of time, not due a greater reliance on equipment, but with wider knowledge and deeper understanding.

We have been delivering this course since 2015 to private clients and are now able to offer it to a wider audience to provide candidates with the skills and knowledge which are appropriate for a non-healthcare professional to deal with advanced injuries and illnesses which one can reasonably expect in remote, austere and hostile environments.

Due Diligence

The two-day (16 hour) course draws on the most recent peer-reviewed evidence for the initial treatment and long-term care of injuries and illnesses in remote areas.  As such, some protocols would not be appropriate in an urban setting.

• National Institute of Clinical Healthcare Excellence

• Joint Royal College of Ambulances Liaison Committee

• Royal College of Surgeons Faculty of Pre-Hospital Care

• Wilderness Medical Society

• Committee on Tactical Combat Casualty Care

• Currently accepted guidance and evidenced-based best-practice from global advisory bodies and peer reviewed publications for the treatment of First Aid in a remote or wilderness context.

Pre-requisites

This is not a First Aid course. The Prolonged Field Care course does not cover Basic Life Support and as such, does not satisfy the criteria for First Aid in the workplace or for National Governing Body Awards coaching qualifications.

It is expected that candidates have previously undertaken a Remote First Aid course or, ideally, a Level 3 Pre-Hospital Care course such as FREC, FPOS or First Responder. Candidates will benefit from a previous understanding of casualty assessment, observing basic vital signs and taking a history.

Part Two - Prolonged Field Care Principles

• Prolonged Field Care - an acronym for long-term care and monitoring

• Long Term monitoring including:

  • Basic vital signs

  • Blood pressure

  • Blood glucose

  • Urinalysis

  • Rapid Diagnostic Testing

• Hydration and Dehydration

• Managing infection

• Sanitation in camp

• Heat Illness and Hypothermia

• Medication - the administration and medico-legal issues

Part 3 - Medical Skills

• Managing Diarrhoea

• Managing Ankle Injuries

• SAM Splint Mastery

• Common Eye Injuries and Illnesses

• Spinal Injuries in Remote Environments

• Emergency Dentistry

• Intramuscular Injection

• Wound Management

Part 4 - Prolonged Field Care Scenario

• Each candidate will assess, diagnose, treat and monitor a different casualty over the two day duration of the course.

Assessment

This course is formatively assessed with continual feedback.

Certification

On successful completion of the course, candidates will receive a Certificate for this 16 hour Prolonged Field Care course, valid for 3 years.

Venue

Training can be delivered here in Pembrokeshire or at your venue.   Because of the practical nature of the course it is ideally delivered with access to the normal or anticipated working environment of the candidates.
